I've played two games in which two other players (presumably cooperating with each other) deliberately avoided killing me when they could have, by just discarding Bang! and other offensive cards instead of playing them. Statistically, if two out of three players refuse to play to win, then there are situations where the game can continue indefinitely: e.g. if the third player never gets a gun or a Cat Balou and the other two both have a Mustang, or if one of the malicious players is Pedro Ramirez and can apply the jail gauntlet. Proposing to abandon the game is not effective if the other players are deliberately trying to prolong the game and force the non-malicious player to quit.
It would be helpful to detect these situations (a player having offensive cards but not playing those cards for many turns in a row) and then give any single player the option to end the game unilaterally without using karma. -
